Dr. Ziny Yen

Lead Family Physician, Limestone FHO
FLA-OHT, PCPC

Dr. Ziny Yen completed her Master’s degree in Medical Genetics at the University of British Columbia before embarking on her long-term medical journey. She completed her medical training at the Queen’s School of Medicine in 2009 and graduated Queen’s Family Medicine residency in 2011.  Dr. Yen has diverse interests in various fields of Family Medicine, with her favorites being in women’s health, pediatric care, minor procedures, mental health and counselling, and chronic disease management.  She truly believes in working as a team along with the patient and their Health Home to achieve the best health outcomes looking at the patient as a whole person and their entire environmental and social context.  
 
In the past 15 years, Dr. Yen’s ongoing mission has been to provide primary care to those in need in the Kingston and surrounding areas. Her dedication to this mission has been seen through her endless work with Operations Management and Administration for the CDK Family Medicine and Walk-In Clinics. CDK’s physicians and PAs have collectively serviced >20 000 patient encounters per year, during times of need, when most walk-in clinics closed practices due to physician shortages.  Most recently, Dr. Yen has been busy recruiting new family doctors for Kingston and helping greater than 6000 unattached patients gain a new doctor, with the help of the entire CDK Family Medicine team, Health Care Connect, OHT, KCHC and City of Kingston.

Dr. Lili Mileva

Associate Lead Family Physician, Limestone FHO

Dr Lili Mileva finished Medical School in Bulgaria where she worked and trained in Cardiology, When she moved to Canada, she finished the Canadian family medicine residency program and started practicing in 2000.

In 2011 she obtained Diploma in Practical Dermatology and additional training in dermoscopy which helps in the diagnosis of skin cancer. Since 2011 along with practicing comprehensive family medicine she has been seeing patients referred by her colleagues for dermatological problems.

Dr. Ola Omisade

Family Physician, Limestone FHO

Dr Omisade obtained her medical degree at the University of Leicester in England in 2013, and finished her training in family medicine in 2018. She then practised as a family doctor in the UK for over 7 years before moving to Canada and joining the clinic in July 2025.
 
She has various areas of interest, but is particularly enthusiastic about preventive medicine and chronic disease management. Her goal is to ensure that her patients receive comprehensive and holistic care.

Dr. Shofia Akhtar

Family Medicine - Physician

About

Dr. Akhtar completed her residency training in Family Medicine at the Northern Ontario School of Medicine in 2021. Prior to residency, she completed medical school in Bangladesh and practised medicine there. Before joining Princess Street Medical Centre in 2024, she also worked in Orillia, Ontario. Dr. Akhtar is passionate about comprehensive family medicine and is dedicated to providing the best possible support to her patients.

Dr. Du Bois’ retirement -notice of Practice Closure

Update for Dr. Du Bois' patients - November 2025

As of Oct 1, 2025, all of Dr. Du Bois’ previous patients have been formally de-rostered from Dr. Du Bois by the Ministry of Health (MOH).  A portion of these patients, especially those with high needs, have been reabsorbed back into the Limestone FHO group by other doctors.  Due to capacity limitations, we were unable to re-attach all patients back to our clinics.

However, please do not lose hope.  I urge you to enroll with Health Care Connect (register at 1-800-445-1822 or online at www.ontario.ca/healthcareconnect), so that you can re-connect with a new family doctor.  Currently, HCC is working hard together with our Ontario Health Team (FLA-OHT) to actively place unattached patients with doctors accepting patients in the Kingston and surrounding areas.  Our FLA-OHT is on track to enroll thousands of patients in the upcoming year.

Limestone FHO has also acquired two new doctors, who also will be enrolling patients from Health Care Connect in the Spring of 2026.  Therefore it is imperative that you register with Health Care Connect to be considered for future attachment to a new doctor in your region.
